I/My/We/Our is denoted as the undersigned/student. The instructor/host of the online course(es) is Brandie Davis/The Davis Protocol of Maternity Massage and shall be abbreviated Brandie Davis/DPMM for the purpose of this agreement.
I understand that this agreement is for any/all DPMM online course(es).
I understand that I am learning infant massage in order to share nurturing and compassionate touch with my child/dependent. If my child or I experience any pain and/or discomfort during any of the course(es), it is my own responsibility, and not that of Brandie Davis/DPMM, to adjust, slow or stop my/our activity and to always conduct the massage in a safe, nurturing environment.
I understand that the physical and mental exercise(s) taught by Brandie Davis/DPMM or performed by me in the online course(es) should not be construed as the substitute for medical examination, diagnosis or treatment, and I/my child or dependent should seek qualified medical assistance for any physical or mental ailment that I/we am/are aware of.
I/We understand that Brandie Davis/DPMM is not qualified to perform spinal or skeletal adjustments, diagnose, prescribe or treat any physical or mental illness, and that nothing said in this online course(es) should be construed as such.
Because massage and/or exercise is contraindicated (should not be done) under certain medical conditions, I/we affirm that I/we have consulted my/our child's/dependent's healthcare professional prior to participating and/or I/we am/are participating with my child/dependent by my/our own choice in this online course(es)/exercises. I/we understand that there shall be no liability on Brandie Davis's/DPMM's part, and that I/we am/are responsible to seek professional advice for my/our child/dependent for any reason.
I/we understand that I/we am/are responsible for my/our own education, and that skipping/not completing any portion of the online course(es) does not grant me a refund/makeup courses.
I/we understand that all sales are final.
I/we understand that any/all of Brandie Davis/DPMM online course(es) are for personal use only and does not certify or license me/us to perform massage on anyone other than my own child/dependent. I/we agree that I/we will not perform massage on anyone other than my own child/dependent and I/we understand that it is illegal to receive monetary compensation for performing massage without proper training/education and a state and/or a national certification/license.
I/we understand the copying, recording, or distributing this course without written permission is strictly prohibited and punishable by law.
